A TODDLER was overdosed on seizure medication when a prescription was incorrectly labelled with a dosage 10 times the required amount.
The prescription bottle was meant to say 1ml but through error it was printed as 10ml, causing near-fatal symptoms and hospitalisation.
Health Minister Chris Picton said, "my heart goes out to the family for what has been a distressing experience".
"Private community pharmacists on the whole do a great job, but where serious medication errors occur it is incumbent upon the pharmacy to review what occurred and if necessary to be reviewed by the national pharmacy board."
A spokesperson for the Ahpra and the Pharmacy Board said, "a pharmacist has to exercise an independent judgement so the medicine is safe and appropriate for the patient".
