PHARMACISTS will participate in home nursing visits in East Melbourne, under a new program launched by RDNS HomeCare and East Melbourne Primary Health Network (EMPHN).
Dubbed TEAMM-Pharmacist (Timely Enhanced Access to Medication Management), the program aims to improve quality of care and medication safety for older nursing home clients referred for medication management.
A clinical pharmacist will visit them in their home, with the service integrated into the team to streamline access for clients assessed as being at higher risk of medication misadventure.
According to EMPHN ceo Robin White, medication errors occur in up to 40% of this client population, with about 13% of incidents requiring hospitalisation or medical consultation.
The TEAMM program is based on a pilot conducted by RDNS and Monash University in 2014/15.
