THE American Society of Health-System Pharmacists (ASHP) has released new guidelines on "Preventing Medication Errors in Hospitals".
The society recommends that pharmacists participate in multidisciplinary committees and take an active role in the evaluation and monitoring of the medication-use process throughout their hospital.
Pharmacists are encouraged to examine and improve systems with a view to enhanced safety.
The stated purpose of the guidelines is to provide pharmacists with practical recommendations and best practices for preventing and mitigating patient harm from medication errors in the health-system setting - see ajhp.org.
