MEDICATION review company Ward Medication Management has launched a new "world first model of care," which aims to reduce the risks of harm by providing individuals with ongoing personalised medication plans based on DNA analysis.
Dubbed "Thrive," the high-touch clinical pharmacy service is being debuted together with aged care providers Allity & Uniting AgeWell.
"Thrive reduces the risks of harm caused by taking multiple medications through DNA analysis, in-depth collaboration with an individual's full health care team, regular medication plan updates and around-the-clock access to a team of clinical pharmacists specialising in geriatric medicine," the company said.
A simple DNA swab is used to measure which medications an individual's body can metabolise well, with the information feeding into the creation of each patient's personal medication plan.
Ward MM CEO Fiona Rhody-Nicoll said Thrive aimed to reduce the alarming number of drug-related hospital admissions in Australia, and also highlights the critical importance of clinical pharmacists within the healthcare system.
The "precision medicine" initiative will roll out nationally from Jan 2019, and will be available from $52 per week for aged care home residents and $72 for those living independently.
