The American Pharmacists
Association (APhA) Foundation has
launched a new “self management
tool” which aims to improve
healthcare outcomes for diabetes
patients, including a national
network of pharmacist providers.
The ‘Patient Self-Management
Solutions’ system is claimed to
reduce the cost of care for diabetes
patients by about $1,000 annually.
It aims to enable pharmacists
to gain a “comprehensive
understanding of their patients’
ability to manage their diabetes...
providers can then customise
and effectively direct treatment
in patient populations,” said a
